Treatment options

Hormone therapy is one option, not the plan. Every page below tells you what the evidence supports, what it does not, and how we decide.

Systemic hormone therapy

Estrogen, with progesterone if you have a uterus. The most effective treatment for hot flashes and night sweats, and it protects bone.

Includes: Estradiol, Transdermal estradiol, Conjugated estrogens, Micronized progesterone

Vaginal estrogen and local therapy

Low-dose local treatment for dryness, painful sex, urgency, and repeat UTIs. Guideline-backed, and barely absorbed into the bloodstream.

Includes: Estradiol vaginal cream, Conjugated estrogens vaginal cream, Estradiol vaginal insert, Estradiol vaginal ring

Non-hormonal options

Prescription treatment for hot flashes without estrogen, plus bladder medication and CBT. Tested against placebo, not a consolation prize.

Includes: Fezolinetant, Elinzanetant, Paroxetine, Venlafaxine

Bone health, DEXA, and osteoporosis treatment

Bone density scanning, vertebral fracture assessment, FRAX risk scoring, and the full range of osteoporosis treatment.

Includes: Dual-energy X-ray absorptiometry, Vertebral fracture assessment, FRAX, Alendronate

Pelvic floor and bladder care

Physical therapy, bladder training, PTNS, bladder Botox, and sacral neuromodulation — in order, least invasive first.

Includes: Pelvic floor muscle training, Bladder training, Percutaneous tibial nerve stimulation, OnabotulinumtoxinA

Testosterone for women

One evidence-based use: distressing low sexual desire after menopause. Everything else claimed for it is not supported, and we will say so.

Includes: Testosterone, Transdermal testosterone gel, Total testosterone, Sex hormone binding globulin

How we rate evidence

Every treatment page carries evidence tags. Strong means randomized trials and current specialty guidelines support the claim. Moderate means the evidence is real but has meaningful limits or mixed results. Limited means early, small, or indirect evidence — and where we rate something Limited, that is us telling you not to count on it.
